Collection of Personal Health InformationWe will collect your personal health information directly from you, or from the person acting on behalf of you. Information we collect will include your name, address, date of birth and detailed health history. We will also record the treatments of every visit, testing/assessment, as well as any home care and exercises. We may need to collect your personal health information from other sources with your written consent, or if the law permits.
Uses & Disclosure of Personal Health Information
We may use and disclose your personal health information for the following purposes:
1. Design a safe and effective personalized treatment plan for you
2. Establish and maintain communication with you
3. Comply with legal and regulatory requirements
4. Fulfill other purposes permitted or required by law
Your Rights
You may access and correct your personal health records, or withdraw your consent for any of the above usage or disclosure.
Our Responsibilities
We will take steps to protect your personal health information from theft, loss, unauthorized access, copying, modification, use, disclosure and disposal. We will conduct audits to monitor and manage our privacy compliance. We will ensure that everyone who performs services for us protects your privacy and only uses your personal health information for the purposes for which you consented.
Policies
AppointmentsAppointments can be booked online or by phone. Same day appointments can be made but a credit card number must be provided and our cancellation policy will be in effect immediately. We do advise you to book as far in advance as possible to receive your preferred choice of times and treatments. As an added convenience we have both male and female registered massage therapists. Unless you state a personal preference, we will schedule your appointment depending on availability.
Upon Arrival
We ask that you please place cell phones and other mobile devices on silent mode because the environment at Tuina.Ca is one of quiet and relaxation. You are also required to complete a health history profile/release form. This is in accordance with the CMTO (College of Massage Therapy of Ontario). This form will also allow our Registered Massage Therapist to be aware of any medical needs or conditions so that the appropriate consideration and modifications can be made during treatments.
Lateness
If you do happen to arrive late for your appointment, we will try our best to accommodate your full booking. However, if that's not available, you will be billed for the full cost of the original booking.
